  1. Where are you currently located?

  2. How many years of outpatient orthopedic experience do you have?

  3. What clinical training/expertise do you have?

    *Include official courses or unofficial training from clinical rotations or other mentorship (ex. SFMA, MDT, Mulligan, etc.). You may include any general areas you excel at (ex. manual therapy, strength training, running analysis, etc.)

  4. Submit a short 3–5 minute video answering the following:

  • Where do you see yourself in 3–5 years?

  • Why are you the right fit for us?

  • Why are we the right fit for you?

*Presentation/editing of the video is not important, content of what you talk about in the video is. A casual unformatted selfie video is sufficient.
